We are an approved Medicaid provider in Arizona and we also provide private pay services for those that are not eligible for Medicaid.

We provide supervised home visits to ensure our clients are satisfied with their care. We are available 24 hours a day to assist you. Our attendants must pass a background check and meet all state & federal requirements to become a personal home care services attendant, including current certifications in CPR & First Aid.
Steps to Receiving Zhon Homecare
Referrals – Referrals are made by CHR’s, discharge planners, physicians, Community Health Nurses and other concerned parties.
ALTC Application – The Arizona Long Term Care Services (ALTCS) program is for individuals who have been approved for Medicaid. However, program participants do not have to reside in a nursing home.
Eligibility – The income limit for this program is $2,000 per month. Persons with Arizona Health Care Cost Containment System (AHCCCS) approved income may have income in excess of $2,313 if held in trust.
Home Assessment – A State ALTCS Representative will make a home visit to determine if you are eligible for ALTCS by meeting health and financial requirements.
Home Care Services – The case manager will allow clients to choose a home care agency to provide an attendant to fulfill the duties determined necessary by the case manager. If a client chooses Zhon home care, a client service coordinator will visit the client a minimum of once a quarter to ensure that all services are being received and that the client’s needs are being met.
Final Step – The client’s referral is submitted to Zhon Homecare billing department. Client intake and attendant hire package is completed. Once services begin, timesheets are submitted weekly to the Zhon Homecare office every Saturday by 5:00 p.m. Client services coordinators, compile all timesheets and complete the TSCOL every Monday. The billing department bills each timesheet and payroll enters hours for the attendants to be paid for their services to the clients.
